Federal officials are now saying issues with both staffing and technology may have contributed to the catastrophic collision between an Air Canada regional jet and a firetruck at LaGuardia airport that killed two pilots. "There are multiple, multiple layers of defense built in to prevent an accident, so when something goes wrong, that means many, many things went wrong,” NTSB Chair Jennifer Homendy said.
