The "inadequate design" of the Titan submersible was a "primary contributing factor" in its implosion, a U.S. Coast Guard Marine Board of Investigation report has found.
The 2023 implosion that killed five people off the coast of Newfoundland was preventable, it concluded.
The submarine, made by Oceangate, went on a dive to the wreckage of the Titanic but never returned. The report says other factors included the company’s critically flawed safety practices, an inadequate design and a toxic work culture.
